Tesamorelin is a synthetic analog of growth hormone-releasing hormone (GHRH) designed to stimulate the body’s natural production of growth hormone. By activating GHRH receptors in the pituitary gland, tesamorelin increases growth hormone secretion and subsequently raises circulating levels of insulin-like growth factor-1 (IGF-1).
Tesamorelin has been extensively studied for its effects on body composition, particularly visceral adipose tissue (VAT), the fat stored around internal abdominal organs. Clinical trials have demonstrated that tesamorelin can reduce visceral abdominal fat in adults with HIV-associated lipodystrophy.
How does Tesamorelin work?
Tesamorelin acts as a GHRH analog, binding to receptors involved in the regulation of growth hormone release from the pituitary gland. Increased growth hormone secretion leads to downstream changes in IGF-1 and metabolic signaling. Clinical research suggests that this pathway can preferentially influence visceral adipose tissue while producing measurable changes in body composition.
How is Tesamorelin Used?
Tesamorelin is administered by subcutaneous injection and has been studied primarily in adults with HIV-associated lipodystrophy and excess abdominal visceral fat. The FDA-approved formulation, EGRIFTA SV, is specifically indicated for reducing excess abdominal fat in this population and is not indicated for general weight-loss management.
Benefits:
Visceral Fat Research: Clinical trials have demonstrated meaningful reductions in visceral adipose tissue following tesamorelin treatment. In a large randomized study, visceral adipose tissue decreased by approximately 15% over 26 weeks compared with an increase in the placebo group.
Body Composition: Research has shown reductions in visceral abdominal fat along with changes in other measures of body composition. A recent systematic review and meta-analysis of randomized trials also reported reductions in visceral adipose tissue, trunk fat, and waist circumference, together with an increase in lean body mass.
Abdominal Fat Reduction: Tesamorelin has demonstrated particular activity against visceral rather than subcutaneous abdominal fat. In clinical research, reductions in visceral fat were accompanied by improvements in measures such as waist circumference and abdominal appearance.
Liver Fat Research: Clinical research has also investigated the relationship between tesamorelin and ectopic fat accumulation. One randomized study reported a reduction in liver fat alongside a reduction in visceral adipose tissue, although further research is needed to determine the long-term clinical significance of these findings.
Growth Hormone Research: Because tesamorelin stimulates endogenous growth hormone production, it is also an important research compound for studying the GHRH–growth hormone–IGF-1 pathway and its effects on body composition and metabolism.
Research Considerations:
Tesamorelin has substantially more human clinical evidence than many experimental peptides. However, its established indication is specific: reduction of excess abdominal fat in adults with HIV-associated lipodystrophy. It should not be presented as a general-purpose weight-loss treatment.
Because tesamorelin increases growth hormone and IGF-1, treatment can be associated with elevated IGF-1 levels, fluid retention, glucose intolerance or diabetes, injection-site reactions, hypersensitivity reactions, and other adverse effects. The FDA labeling also contains warnings concerning malignancy risk and notes that long-term cardiovascular safety has not been established.
Dosage:
The FDA-approved EGRIFTA SV formulation has a specific dosing regimen; however, dosing should not be generalized to other tesamorelin products, formulations, or research preparations. The FDA labeling specifies that its dosing recommendations apply specifically to the approved 2 mg-per-vial EGRIFTA SV formulation.
Conclusions:
Tesamorelin is a synthetic GHRH analog with well-established research involving growth hormone signaling, visceral adipose tissue, and body composition. Clinical trials have demonstrated its ability to reduce excess visceral abdominal fat in adults with HIV-associated lipodystrophy, making it substantially more clinically characterized than many emerging peptide compounds.
At the same time, tesamorelin’s effects on growth hormone and IGF-1 require appropriate medical consideration, and its FDA-approved use is limited to a specific patient population. It should not be marketed as a general weight-loss solution.
Tesamorelin is presented here for research and educational purposes and should not be considered a substitute for professional medical advice or an approved treatment outside its authorized indication.
