Semax is a synthetic heptapeptide derived from the ACTH(4–10) sequence and is classified as a neuroactive peptide. Developed in Russia, Semax has been investigated for decades in neurological and cognitive research, with studies exploring its potential effects on neuroprotection, cognitive performance, neuronal signaling, and recovery following neurological injury.
How does Semax work?
Semax is believed to influence several pathways involved in neuronal function and cellular adaptation. Preclinical research has particularly focused on its relationship with brain-derived neurotrophic factor (BDNF), a protein involved in neuronal survival, growth, and synaptic plasticity. Animal studies have reported changes in BDNF-related signaling following Semax administration, while some human research has investigated changes in circulating BDNF and neurological outcomes.
How is Semax Used?
Semax has traditionally been investigated and used in intranasal formulations, particularly in Russian clinical research. Studies have examined the peptide in the context of ischemic stroke recovery, cognitive impairment, attention, and other neurological conditions. However, evidence from these studies should be interpreted cautiously because much of the clinical literature consists of relatively small Russian-language studies, with limited independent replication in large, modern randomized controlled trials.
Benefits:
Cognitive Research: Semax has been studied for its potential influence on attention, memory, and other aspects of cognitive performance. Early human research has reported changes in attention and short-term memory, although the available evidence remains limited.
Neuroprotection: Semax has attracted considerable research interest for its potential neuroprotective properties. Preclinical studies have investigated its effects on neuronal survival, cellular stress responses, and mechanisms associated with neurological injury.
BDNF & Neural Signaling: Research has examined Semax in relation to BDNF and other neurotrophic signaling pathways. A clinical study involving patients recovering from ischemic stroke reported increased plasma BDNF levels following Semax administration, although this does not by itself establish that BDNF changes directly caused improved recovery.
Neurological Recovery Research: Clinical studies conducted primarily in Russia have investigated Semax as an adjunct in neurological rehabilitation, particularly following ischemic stroke. Published findings have reported improvements in certain neurological outcomes, but the overall certainty of evidence remains low or very low because of limitations in study design and independent replication.
Cognitive & Brain Research: Semax has also been examined in healthy volunteers and experimental models to better understand its effects on brain activity and cognitive processes. These studies provide useful mechanistic information but are not sufficient to establish Semax as a proven cognitive-enhancement treatment.
Research Considerations:
Semax has a longer history of human investigation than many newer research peptides, but its evidence base has important limitations. Much of the published clinical research originates from Russia and is not supported by large-scale Western randomized controlled trials. A recent evidence synthesis rated the certainty of evidence for cognitive performance and neuroprotection as very low, primarily because of risk of bias and reporting limitations.
Regulatory status also varies by country. Semax is authorized for clinical use in Russia, but it is not FDA-approved in the United States. In July 2026, the FDA’s Pharmacy Compounding Advisory Committee review also stated that there was insufficient clinical information to characterize the safety of intranasal Semax, no published human pharmacokinetic studies, and no safety data for subcutaneous administration.
Dosage:
There is no FDA-approved general human dosing regimen for Semax in the United States. Dosing protocols reported in published research vary according to the indication, formulation, route of administration, and study design. These experimental protocols should not be presented as established recommendations for general human use.
Conclusions:
Semax is an interesting neuroactive peptide with a substantial history of research, particularly in Russia. Its potential effects on cognitive function, neuroprotection, BDNF signaling, and neurological recovery have generated continued scientific interest.
Although some clinical studies have reported encouraging findings, the overall evidence remains limited by small study populations, methodological limitations, and a lack of independent large-scale replication. Additional well-designed human clinical studies are needed to establish its safety, efficacy, pharmacokinetics, and potential therapeutic applications.
Semax is intended for research purposes and should not be considered a treatment or cure for any medical condition.
